Liability Waiver and Acknowledgment of Risks for Use of the Water Attraction Park

Before visiting the water inflatable park, this waiver must be completed and signed.
Only an adult (18 years or older) is authorized to sign this document. For minor participants, the form must be completed and signed by their legal representative (parent or guardian), who assumes full responsibility.

! IMPORTANT!

This is a single-use waiver valid only for the specific visit. A new form must be completed before each new use of the water park.

By signing below, I CONFIRM THAT:

1. I have read, understood, and agree to comply with the safety and internal regulations of the “Marupe Wake” water inflatable park.
2. I acknowledge that using water attractions is physically demanding and potentially dangerous, and may result in injury, health complications, or even death. I fully understand and accept these risks.
3. I have no health issues that would prevent or restrict my participation. I am aware of my own physical abilities.
4. I assume full responsibility for any physical injuries or harm to myself, others, or third parties resulting from:
   – non-compliance with safety regulations;
   – misjudgment of my health or capabilities;
   – negligence or inaction;
   – or any other act on my part.
5. I accept full responsibility for any delayed injuries or health complications that may arise at any time after using the water park.
6. I also assume responsibility for any damage or losses caused to third parties due to my actions or failure to act, or due to violations or inadequate compliance with safety rules.
7. SIA “Beng” (“Marupe Wake”) is not liable for any losses or claims arising from my actions or inactions. I waive any claims against the company related to my participation.

IF THIS FORM IS COMPLETED ON BEHALF OF ONE OR MORE MINORS:

8. As a parent, legal guardian, or authorized representative, I confirm that all minors listed below have been informed of the rules and are aware of the safety requirements.
9. I accept full responsibility for each child’s safety, behavior, health, and life during the entire operating season.
10. If I am not the legal guardian, I confirm that I have the appropriate authorization from the child’s parent or guardian.
11. By signing, I confirm that I am at least 18 years old, I have read this waiver, understand it fully, agree to all of its terms, and confirm that the information provided is accurate and true.
