Minor Participant Waiver and Consent – Wakeboarding

WAKEBOARD PARK PARTICIPATION WAIVER FORM FOR MINOR

Before a minor participates in wakeboarding activities, this waiver form must be completed and signed. This form is mandatory only in cases where the minor visits the park alone, without the presence of their legal representative (parent or guardian).
Only an adult (18+ years old) who is the legal representative (parent or guardian) of the minor is authorized to sign this document.

By signing below, I HEREBY CONFIRM THAT:

1. I have read, fully understand, and accept the internal rules and safety regulations of the “Marupe Wake” wakeboard park, and I agree that the minor(s) listed will comply with them.
2. I understand that wakeboarding is a physically demanding and potentially dangerous activity that may cause injuries or pose health risks, including the risk of death. I fully acknowledge and accept the inherent dangers of this activity.
3. The minor(s) have no health issues that would restrict or prohibit their participation in wakeboarding activities.
4. I take full responsibility for any physical injury or harm to the minor(s), others, or third parties resulting from: failure to follow safety rules; misjudgment of health or abilities; negligence or inaction; or any other actions by the minor(s).
5. I take full responsibility for the health and life of the minor(s) in case of injuries or trauma that may occur or become apparent at any time following participation in the wakeboard park.
6. I take full responsibility for any damage or losses caused to third parties as a result of the actions and/or inactions of the minor(s), including any violation or improper application of safety rules.
7. SIA “Beng” (“Marupe Wake”) is not liable for any losses or claims arising from the actions or inactions of the minor(s). I hereby waive any claims against the company in relation to the minor(s)’ participation in the activity.
8. As a parent, guardian, or legally authorized representative, I confirm that all minors listed in this form have been informed of the rules and understand the safety requirements. I accept full responsibility for each child’s safety, behavior, health, and life.
9. If I am not the legal guardian of the child(ren), I confirm that I have proper authorization from the parent or legal guardian.
10. By signing this document, I confirm that I am at least 18 years old, have read and understood this waiver, agree to all its terms and conditions, acknowledge my obligations, agree to comply with the rules, and declare that all the provided information is true.
